Stormwater Inspection Form Submission Instructions
If you do not have an email address, then you must complete and submit a paper form.  Please exit this form and download the appropriate form from the Arlington County web site.
Next, refer to the letter that was sent to the facility owner for requested information.  Owner, facility and the inspection form(s) required by each facility are noted there.  You must complete one inspection form for each facility listed.
The electronic inspection form consists of several pages (listed on the left).  Enter the information or complete the tasks described on each page.

A description of each page and the information it contains is as follows:
- Form/Facility Verification - Enter the Form ID (noted on the letter you received) and verify Facility information.
- Owner/Agent Verification - Review, and/or update Owner/Agent information.
- Inspector Information - Enter information regarding the inspector.  A professional inspector may be required to perform the inspection; if this is the case, either a professional inspector must complete this online form, or the owner/agent must provide a copy of the Inspector's report/invoice on the Photos / Attachments page. 
- Inspection Tasks - Complete the inspection questions.
- Component Details - View any diagrams that may be associated with the type of inspection form selected (if applicable).
- Photos / Attachments - Upload photos and other attachments on the Photos page.  You may attach up to six (6) additional photos/attachments other than those required if you wish.
- Sign & Submit - Sign and submit your form once you have completed all questions and uploaded all required photos/files.

Vegetation Tasks

Inspect the surface for weeds.  Remove if present.

a) Ensure that all of the plants are healthy.  Replace any that are dead, dying or diseased.

Inspect the condition of the trees or shrubs, prune as necessary.

Mulch Tasks

1.  

Ensure the mulch layer is uniform in depth (2-3 inches) and level. Rake if necessary. 

Annual Inspection

1.  

Inspect the gutters and downspouts for leaves and debris. Remove any that have accumulated. This should be completed twice a year. 

2.  

Inspect the prescreening and first flush diverters for leaves and debris.  Remove any that have accumulated.  This should be completed four times a year.

3.  

Inspect the vents and screens on inflow and outflow spigots. If there are any holes, these should be patched.

Every Third Year Inspection

1.  

Inspect tank for any sediment buildup.

2.  

Inspect the overhanging vegetation or trees.  Prune any that are overhanging the roof.

3.  

Inspect the backflow preventer.

4.  

Inspect the tank, pump, pipe and electrical system for any needed repairs.

1. 

Examine curb cuts, gravel diaphragms or pavement edges for any sediment buildup that prevents flow from getting into the channel and check for other signs of bypassing.  Remove sediment if present.

Structural Components

1.  

Is the planter box structurally sound*? 

(Planter boxes only.  If repairs are made, explain in the Notes section at the bottom of the form.)

Inspect the observation well or overflow pipe to ensure covers are in place.

Examine curb cuts, gravel diaphragms or pavement edges for any sediment buildup that prevents flow from getting into the dry swale and check for other signs of bypassing.

Examine for evidence of excessive ponding or concentrated flows.  If present, identify and eliminate the source of the problem.

5.  

Check for clogged or slow-draining soil media, a crust formed on the top layer, inappropriate soil media, or other causes of insufficient filtering time, and restore proper filtration characteristics.  If necessary, remove and clean or replace the clogged material.

6.  

Inspect upstream and downstream of check dams for evidence of undercutting or erosion. Remove trash or blockages at weepholes.

Vegetation Tasks

1.  

Inspect the vegetation for any weeds or leaf accumulation. Remove if present.

2.  

a) Ensure that all of the plants are healthy and that at least 80% of the roof has plant coverage.  Any that are dead, dying or diseased need to be replaced.

Ensure that there is 95% turf cover or vegetation density has been achieved in the bed and banks of the dry swale.

2.  

a) Ensure that all of the plants are healthy.  Any that are dead, dying or diseased need to be replaced.

Ensure that the contributing drainage area is stabilized.

Repair undercut and eroded areas at inflow and outflow structures.

Invasive Plant Management (IPM)

IPM - General Tasks

1.

a) Inspect management area for target non-native invasive species.

b) If target species are not identified, inspect management area for any non-native invasive plant listed on the current Non-Native Invasive Plants of Arlington County list. 

 c) Remove non-native invasive plants, if present.

2.

a) Inspect management area for non-native invasive vines on trees.  Invasive species include English Ivy, Japanese/ Chinese Wisteria, Oriental Bittersweet, Climbing Euonymus, Porcelain Berry and Japanese Honeysuckle.  

 b) Remove invasive vines, if present.

3.

a) Inspect management area for invasive species on the ground.

b) In addition to the above vines, ground species include bamboo, Chinese privet, multiflora rose, vinca, porcelain-berry, bush honeysuckle, and Japanese barberry.

c) Remove invasive species, if present. 

IPM - Vines in Trees

1.

a) Inspect management area for non-native invasive vines on trees.  Invasive species include English Ivy, Japanese/ Chinese Wisteria, Oriental Bittersweet, Climbing Euonymus, Porcelain Berry and Japanese Honeysuckle. See more invasive species here.   

b)  Remove invasive vines, if present.

IPM - English Ivy

1.

a) Inspect management area for non-native invasive English Ivy.

b)  Remove English Ivy, if present, from both the ground layer and trees.

1.  

Ensure the level spreader surface is level and in good condition (i.e., has not settled, been undercut, eroded). 

2.  

Ensure the stone surface of the level spreader is free of vegetation and excessive sediment accumulation.

3. 

Ensure the area behind the level spreader is free of erosion and sediment.

1.  

Inspect contributing drainage area for any controllable sources of sediment or erosion. 

2.  

Inspect the structural integrity of the pavement surface, looking for signs of surface deterioration, such as slumping, cracking, spalling* or broken pavers.

3. 

Inspect the surface for evidence of sediment disposition.  Any areas that are clogged need to be vacuumed.

4.  

Inspect the surface for evidence of organic debris.  Any areas that have accumulated leaves or mulch need maintenance.

5.

Check for vegetation growing between pavers. Remove/pull all vegetation if present.

6. 

Inspect the surface for staining or ponding that may indicate surface clogging.  Any areas that are clogged need to be vacuumed.

Observation Well Tasks  (if applicable)

1.  

Inspect the condition of the *observation well (pipe cap in driveway) and make sure it is still capped.

2.

 a) Open the cap on the observation well and measure the amount of standing water in the observation well. 

 b) If standing water is found, then perform maintenance.

1.

a) Inspect upstream and downstream of check dams for evidence of undercutting, side cutting or erosion.  
b) Repair immediately.

6. 

Inspect downstream of check dams and remove any accumulated sand or sediment deposit.

Vegetation Tasks

1.

Ensure that there is 90% turf cover in the channel and banks. Re-establish if necessary.

2.

Ensure that the turf is healthy.  Treat and remove invasive species and weeds.

General Tasks

1.  a) First Chamber is to be pumped out
     semi-annually.
2.  When drawdown exceeds 72 hours,
     upper layer to be replaced.

Ensure roof drain covers are present.

Ensure roof drain covers are in good condition.

Remove any debris (including leaves).

Replant/reseed any damaged vegetation, add reinforcement plantings to maintain 95% native vegetated cover.

a) Identify and control invasive plants.

Remove sand or sediment deposits along low flow channel if more than 50% of the stream bed surface area is displaced by sediment or if step pool structure is not visible.

4. 

Inspect upstream and downstream of all step pool wall for evidence of undercutting or erosion and remove all trash or blockages.

5.

Examine stream bed for evidence of braiding, erosion, cobble mobilization or excessive scour in step pool areas.

6.

a) Check inflow points of stilling basin and step pools for clogging.  

b) Remove excess sediment if more than 30% of step pool or stilling basin depth or volume is displaced by sediment.

7. 

Inspect side slopes for any evidence of rill or gully erosion and repair as necessary.

8. 

a) Inspect step pool structures for stability. 

b) Repair if 30% or more of the structure has failed.

1. 

Measure sediment accumulation levels.  

(Make sure they match approved plan elevations.)

2.

Inspect the condition of stormwater inlets and channels to the pond for material damage, erosion or undercutting.

3. 

Inspect upstream and downstream banks for evidence of soughing, animal burrows, boggy areas, woody growth or gully erosion that may undermine embankment integrity.

4. 

Inspect the pond outfall channel and emergency spillway for erosion, undercutting, rip-rap displacement, woody growth, etc.

5.

Inspect condition of principal spillway and riser for evidence of spalling, joint failure, leakage, corrosion, etc.

6. 

Inspect condition of trash racks for evidence of clogging, leakage, debris accumulation, etc.

7. 

Inspect internal and external pond side slopes for evidence of sparse vegetative cover, erosion or slumping, and repair immediately.

Tree and Shrub Tasks

1. 

Ensure trees and shrubs are present per the planting plan and are in good condition.

2.

a) Replace missing, dead or dying trees and shrubs per the planting plan.

b) If any trees or shrubs were removed or replaced, please note the number and type of those in the Task 2c box below.

3. 
Free trees and shrubs from non-native invasive vines. See a list of invasive species here.

4. 
a) Ensure plants less than two (2) years old are being watered during periods with less than one (1) inch of precipitation per week.

b) Ensure plants two years old or more are being watered during droughts.

5. 

a) Ensure or place deer protection on trees (if required per plan).

b) Ensure existing deer protection is in good condition.

c) Remove deer protection from trees two (2) inches in diameter or larger.

Perennials and Groundcover Tasks

1. 

Ensure perennials are present, in good condition, and provide at least 80% coverage of the area. 

2. 

a) Replace dead perennials and plant as needed to cover 80% of the area.

b) Document the number and type of plants replaced in the Task 2c box below.

3. 

Ensure planting area is free of non-native invasive plants and other weeds. See a list of invasive species here.

4. 
a) Ensure plants less than two (2) years old are being watered during periods with less than one (1) inch of precipitation in a week.

b) Ensure plants at least two (2) years old or older are being watered during droughts.

5. 
Cover bare soil with 2-3 inches of undyed, double-shredded hardwood or leaf mulch.

1. 

Inspect and clean gutter screens/gutters.

2. 

Inspect and clean inflow screen. (Perform this task monthly and after heavy rainfall.)

3. 

Observe pipe connections for leaks.

4. 

Inspect sump and remove drain plug to remove any debris, if necessary.

5. 

Inspect outflow for erosion and/or clogging.

Inspect the tank's structural integrity and pipework by qualified professional(s). (Perform once every 5 years.)
